Double click on the .csh file and they’ll install themselves to your Photoshop custom shapes tool. (You can access this tool by right click on your canvas when using your custom shapes tool.)

All we ask is that you don’t design the exact same shirt and then try to sell them. if you do this Mr. 51% will have to hunt you down and do scary things to you.
